i've come to a stage of plateau and was thinkin about changing up the workout. But do i change one exercise in each muscle group and keep the rest the same, or change all the exercises as well as the reps count.

I would say a change of your entire routine for a period of time would get you over the plateau. Something that will refresh and get your muscles thinking again.

change ya workout around every 8 - 12 weeks keeps it interesting and the muscles working and having a week off every 10-12 weeks as well to let your body recover
